FDA Approves Foundayo (Orforglipron): Lilly’s Game-Changing Oral – In a major win for millions of Americans struggling with obesity, the U.S. Food and Drug Administration has officially approved Eli Lilly and Company’s newest weight-loss medication. Foundayo™ (orforglipron) is now the only once-daily GLP-1 pill that can be taken any time of day, with or without food or water. This approval gives patients a simple, flexible oral option that fits real life—no injections, no strict timing rules, and no more planning your day around a dose.
Lilly’s second FDA-approved obesity treatment, Foundayo is designed for adults living with obesity or those who are overweight with at least one weight-related health condition (such as high blood pressure, high cholesterol, or sleep apnea). When paired with a reduced-calorie diet and regular physical activity, it helps people lose significant weight and keep it off long term.
Why Foundayo Could Be a Game Changer for Obesity Treatment
Many people avoid GLP-1 medications because of the hassle of injections or the need to take pills at specific times with food and water. Foundayo changes that completely. As a small-molecule, non-peptide oral GLP-1 receptor agonist, it works by mimicking natural gut hormones to curb appetite, promote feelings of fullness, and support steady weight loss—without the usual restrictions that make other treatments feel complicated.

Experts are already calling it a practical breakthrough. “People living with obesity need options that fit their daily routines,” said Deborah Horn, DO, director of the Center for Obesity Medicine at McGovern Medical School at UTHealth Houston. “A pill you can take anytime offers real flexibility and could help more patients stick with treatment.”
Strong Clinical Results Back the Approval
The FDA’s decision is supported by impressive data from the ATTAIN Phase 3 clinical trial program, which enrolled more than 4,500 participants worldwide.
In the key ATTAIN-1 trial, adults taking the highest dose of Foundayo who stayed on treatment lost an average of 27.3 pounds (12.4% of body weight) after 72 weeks—compared to just 2.2 pounds (0.9%) for those on placebo. Even when including everyone who started the study, the average weight loss was 25 pounds (11.1%) versus 5.3 pounds (2.1%) on placebo.
Participants also saw meaningful improvements in several heart-health markers, including reductions in waist circumference, non-HDL cholesterol, triglycerides, and systolic blood pressure across all doses.
“Fewer than 1 in 10 people who could benefit from a GLP-1 are actually using one,” noted Eli Lilly Chair and CEO David A. Ricks. “Foundayo was built for the real world—convenient, effective, and designed to reach more people who need help managing their weight and related health issues.”
Easy Access and Affordable Pricing Options
Lilly is making Foundayo available right away through its direct-to-patient platform, LillyDirect®. Prescriptions are being accepted now, with free home delivery shipments starting April 6, 2026. It will roll out shortly afterward to major retail pharmacies and telehealth providers across the U.S.
Affordability was clearly a priority:
- Eligible patients with commercial insurance can pay as little as $25 per month using the Foundayo savings card.
- Self-pay customers can access the lowest dose for $149 per month.
- Eligible Medicare Part D beneficiaries may qualify for $50 per month starting July 1, 2026.

Important Safety Information You Should Know
Like other GLP-1 medications, Foundayo is not recommended for use with other drugs in the same class. It has not been studied in children, and its safety and effectiveness in pediatric patients are unknown.
Important warning: Foundayo may cause thyroid tumors, including a type of thyroid cancer. Watch for symptoms such as a lump or swelling in the neck, hoarseness, trouble swallowing, or shortness of breath, and contact your healthcare provider right away if they occur.
The most common side effects are typically mild to moderate and include nausea, constipation, diarrhea, vomiting, indigestion, stomach pain, headache, feeling tired, belching, heartburn, gas, and hair loss. Your doctor can help manage any side effects and determine if Foundayo is right for you.

A New Chapter in Obesity Care
Joe Nadglowski, president and CEO of the Obesity Action Coalition, welcomed the news: “There’s no one-size-fits-all solution for weight management. More treatment choices like Foundayo mean more people can find an approach that works for their lifestyle and goals.”
Lilly has already submitted orforglipron for approval in more than 40 countries and plans rapid launches following local regulatory clearances. The company is also studying the medication for additional uses, including type 2 diabetes, obstructive sleep apnea, osteoarthritis knee pain, and other conditions.
The Bottom Line
The FDA approval of Foundayo marks a pivotal moment in obesity treatment. For the first time, patients have access to a highly effective oral GLP-1 pill that delivers meaningful weight loss without the inconvenience of injections or rigid dosing schedules.
If you or someone you know is exploring weight-management options, talk to your healthcare provider about whether Foundayo could be the right fit. With convenient access through LillyDirect and competitive pricing programs, help is now easier to reach than ever.
